The first question to ask as to a confession would be "what purpose would it serve". If he is guilt ridden, then he is punishing himself (or should perhaps confess to a minister of sometype). The cheating of course was wrong, but does he improve the situation by confessing to his wife?

Ok does he wants to confess to heal his guilty consious? In the first place, why did he cheated on her? if he didn't because he has no respect nor feelings for her! then he should be honest and tell her but!! he needs to be able to accept the consequences this confessions comes with! But, if he just want to tell her so he could feel better, then he could promise to himself not to ever ever do it again and to be able to keep that promise and also to be nicer to his wife!!. He needs to make that decision himself!!! either way at the end she will find out!! at the end, life has different ways to show the truth!

He has to chose the lesser of 2 evils. Either being without a wife if she leaves...or living a life of guilt.
He may be able to get rid of this guilt if he tells someone or goes to a therapist. He could learn to forgive himself and find out why he did it. She doesn't ever have to know.
However if this method doesn't work...he may have to tell her or he'll slowly be torturing his soul and living an unhappy life.

If you know who this man is and you know his wife, tell him to tell the truth because if one day the wife finds out, things will be worst, and if he didn't use protection, that is the worst thing that he can do to his wife. They say the "truth will hurt" but take my word for it,at least she will know instead of pretending that their life is perfect,...from m,y own experience,

Once a cheater, always one. He should confess, leave, so she can find someone worth of her trust.... when the trust is gone, the relationship is in the toilet.
