I take time out for me and I write a journal. I list what I want to change, I make sure they are realistic goals, then I plan what I need to do to accomplish these goals and then do it.

Excellent question! I'm recently doing that myself.
My new gf has motivated me so much I've started a business, started a website and put my packet in for Warrant Officer school. She never once suggested any of those things, I just did it.
Anyway most people are only concerned with A: present condition and B: desired condition... they forget all about or are not willing to do C: things you have to do to get to B.
Write down A and B and then find out C. Then DO C. It takes action, action, action... NOW DO IT and get off your you know what!

I have very lucid dreams anyway. I suggest studying lucid dreaming techniques. An Ebook helped me I uploaded here (is public domain) *see source link #1*

Also, have started writing allot of my dreams, right after waking. DREAMHOST, *see source link #2* has a new free service for just such a thing. Here's an example of one that has been used more as a splattering of posts from those whom are learning. Provides a few good links too!! * see source link #3*

Hope these help!

I dream... then I make lists. Putting it down on paper makes it *real* for me, and helps me focus. I can then process the pros, the cons, the little additives I'll need to get where I'm going... and then sharing with a respected friend helps as well. Creative problem solving, identifying the puzzle, examining the pieces, and finding solutions to get that *best fit*.

I listen to what God is telling me; if He is for me, nobody can be against me, ultimately :)
