You either have a leaky wax seal, or a leaky tank to bowl gasket, or possibly both.
The continuos sound from the tank is most likely a worn out flapper.
Replace the flapper in the tank.
Make sure the outside of the toilet & bathroom floor are completelt dry, & flush the toilet observing the tank to bowl area, as well as the toilet base for leakage,
Make your repairs as necessary.
Hope ths helped, good luck!!

I agree completely with Daniel S. This is something you should fix quickly, otherwise you may rot the flooring out in your bathroom if it is not already rotted out. I'd crawl under the bathroom and see how the underside of your flooring looks. If the wood looks all darker around the pipe that the comes down from the toilet, you may have to replace the flooring as well. Another good idea is to have the line checked that goes into your house. My grandpa once had his rust through and his water consumption went way up. I'd especially have it checked if you notice your lawn being extra wet.

If it is happening only when you flush (which I doubt) it is probably the seal between the tank and bowl. Remove the tank lid and see if water is continually flowing from the fill-valve. Check the water level in the tank. It should be about 3/4" below the overflow pipe. Check the flapper to see if it is leaking through to the bowl.
