Some additional info would help. I won't say this is a new fish, getting used to his tank for the amount of time you've had him. And apparently you're keeping him in a freshwater environment which is correct.

What size tank is he in? Unfortunately, just saying that you had the water tested doesn't give enough information - what parameters were tested? pH should be neutral to slightly acidic, ammonia and nitrites should be zero - puffers are extremely sensitive to these. If he's in a relatively new tank, with a pleco for a buddy, and the tank is on the small side, the ammonia and nitrite would build up quickly.

The behavior you describe does sound like the swim bladder is affected. This usually comes from an improper diet - usually relying too much on dried foods. Try feeding frozen live foods as recommended in the second source article or soaking any pelleted food in a little tank water before feeding. nicely what do you feed it? puffers could devour snails because their teeth not in any respect end turning out to be. in case your no longer feeding him snails then his teeth are probly over grown and also you need to start up.(via snails i meant like toddler trumpet snail that are those tiny little snails youll see in petstore tank walls and they'll be extra then chuffed to get rid of a few of those)sturdy success!

More info please, but it sounds like a swim bladder infection. Might be worth getting some antibiotics in there asap to cover that base.
