There two main ways to make a natural fist.
1. natural fist with the thumb over the fingers
2. natural fist with the thumb sitting on the side of the index finger.
There are also variations beyond this such as a fist with the middle knuckle extended.
There are also various ways of impacting with the fist. Some systems teach that the impact should be with the index and middle knuckels. Other systems like wing tsun kung fu teach to hit with the smaller knuckles. Then beyond hitting with the knuckles, there are hammer fists which strike with the meaty part of the hand. Other methods of using the fist exist as well.
Back to the basic question.
To make fist number 1:
a) start with an open hand
b) fold the fingers at the middle and outer knuckles
c) fold the fingers at the inner knuckles
d) adjust to make fingers for comfort
e) fold the thumb over the index and middle finger, the outer thumb joint should be bent at 90 degrees.
To make fist 2:
a) start with an open hand
b) fold the fingers at the middle and outer knuckles
c) fold the fingers at the inner knuckles
d) adjust to make fingers for comfort
e) fold the thumb down so that the tip pad of the thumb is resting on the side of the index finger's middle joint.

There are several ways to make a fist. Depends on what you want to strike. Ninjutsu has quite a few. Fudoken is the "classic" clenched fist used for striking areas such as the face, sternum, arm, leg..... etc. Hard surfaces basically. Never tuck the thumb inside the fist as others have said. You WILL hurt yourself.

Just make a TIGHT fist with your thumb outside of your fingers.
Even more important, is to have your target, striking surface of the fist, forearm and elbow in a straight line. Otherwise you might sprain a wrist, break a wrist, tear some tendons or tear some ligaments.

You can make a tighter fist with your thumb on the outside instead of the inside. If you punch someone hard and you are squeezing tightly with your thumb on the inside, you could pop your thumb out of its joint. Ouch!
Fold all of your fingers tightly into your palm. Wrap your thumb over the top, in between the knuckle attached to your hand and the knuckle in the middle of your first finger.

people may make joke on you.
you must be careful to get advise from internet or TV.
please do not put the thumb inside other fingers to make a fist ----- no matter what style, never heard martial artists hold a fist like that.

My son took Karate, Tai Kwon Do, Judo.
He makes his fist with the thumb on the outside. He said that if you put your fist in the palm of your hand and close your hand around it ... you can break your thumb on impact.
You also have to have your hand to wrist straight.

Curl your fingers in from your pinky to your index finger. Your index finger and middle finger knuckles should be farther out than your other knuckles. Those are the two knuckles you want to strike your target with. The other two knuckles are too weak and vulnerable-they break easily.
Keep your fist slightly loose until the moment of impact. As for your thumb, I like to keep it on top of my fist, tight, and sticking out slightly to strike with.

Thumb ALWAYS goes on the outside. Only little kids wrap their thumb with their fingers. Think about how unnatural it feels to have your fingers around your thumb. That leads to injuries. And you're going to hit something with your fist with your thumb like that?
Most people have their wrist wrong too with the fist somewhat tilted upwards. This leads to injuries as well. You want everything to be linear... with your knuckles in line with your wrist, in line with your arm, in line with your shoulder.

Very simple. Put your four fingers in (index, middle, ring, and pinky), and bend them towards the center to your hand. Put your thumb over it. Your thumb should be on the outside. Don't listen to those other guys on "other answers". It doesn't make sense. -_-

without sounding like I'm making a joke of your question, cause I'm not. Just close your fingers together, thumb outside the fingers. Just try it various ways, then ask yourself what worked best for me. Who cares what works for anyone else. I like to curl my fingers in, thumb facing up. Now bring thumb pad down onto index finger. That's just me it works and I like it. For me it feels more stable. Take the answers you get and try them out, the real answers, and see what works for you.
