The Book of Mormon tells of two different civilizations; one group that left Jerusalem at the time of the Tower of Babel and the second group in 600 B.C.
This book describes the histories, both secular and non-secular, and focuses strongly on the mission and divinity of Jesus Christ.
The last prophet of the latter civilization recorded his words on gold plates in the year 421 A.D.
This book was translated into English by Joseph Smith and published in April 1830 for the world to read.

No. The time between Malachi (last Old Testament book) and Matthew (first New Testament book) is about 400 years.

There is no book between the Old and New Testament. Between the testaments, there were 400 years of silence from God. No prophets, no amazing wonders, just silence until John the Baptist came along.

Yes, there is. The Book of Mormon begins with the journey of a prophet (Lehi) and his family from Jerusalem during the time of Jeremiah the prophet. It begins at about 600BC and ends (I think) somwhere around 300AD, I believe. Excellent book.
