If not a vacation, does she love music? Get her some nice music equipment. After 6 years, though, I would be looking for that engagement ring.

What about an engagement ring? Aren't 6 years enough to realize she's the one? I doubt there is a gift more precious than this one to make her feel happy.
If not, pretend you forgot about her bday and tell her very seriously "can you join me next week-end to a place? a colleague is celebrating smth and he invited me, so I can not go alone". And plan a romantic getaway to a place with a great landscape, book a cottage for the 2 of you and spend a beautiful time over there...And buy her a beautiful dress or a sensual lingerie.

Having maid service clean her house for a month/year? Fresh flowers sent for a year? A full meal delivered from favorite restaurant once a week for a month/year? A week getaway w/ her favorite person(other than you) for a girls getaway. If she is as busy as myself, anything to do w/ relieving some of the burden of everyday chores sounds heavenly....

My GF loves the office (us version) so i went to cafepress and made her a shirt with her favorite catchphrase on it cost like 20 bucks and showed that i really was paying attention. she wears it all of the time.

How about a vacation? You could take her somewhere beautiful and that way you could spend that time together, and you get to enjoy the present too!
