Wow - only one city - that's like saying which one of your children you'd keep! There are so many beautiful places with their individual charm. I would have to agree that Rome is an absolute must, so if you can only choose one, that would have to be it. With a week, though, I think you have enough time to squeeze in an overnight train somewhere else - a second would possibly be Florence but I would recommend Venice. I know these are the ones all tourists talk of, and are overrun with tourists (and cheap souvenirs) but they really are spectacular, with many different sights to see.

Probably the most beautiful place I found in Italy was a small village in the mountains but it, like many of the spectacular smaller places, would not have held me for a whole week.

Rome. because i have been there and its beautiful and jaw dropping. all its tourist sites such as the colosseum, spanish steps, fountain of trevi, the vatican city etc.. are worthing seeing.
i recomend to do all the tourist things.
shops are great, the food and the people are fantastic.
you will love it and will want to go back again

Thats a tough question but i will have to say Rome. So much to see, so much to do. Amazing city.
