Even using a size 16 needle I can't get even stitches over the thickest part of the hem, and the needle gets stuck.

Even my industrial machine gets balky on too many denim layers! If you are doing the standard, fold press, fold press stitch hem, then trimming away bulk from the seams is the way to go. There's little stress on the seams at the hem, so trimming away most of the seam allowance won't harm the garment. If it's s single turn, again, try to trim away as much bulk from the seam as possible. Trim away excess bulk whenever seams or hems cross each other. And even though you are using an 16, is it a needle specially made for denim? These needles have sharp, chisel points to peirce the many denim layers. In a real pinch, try a leather needle, these have points that cut the fabric. Be warned, leather needles will cut permanent holes in fabric and when jeans finished this way are washed, sometimes frayed out holes can form.

If you are into spending some $$$$$, you might consider the purchase of what is called a "Jeans machine"....a sewing machine made especially for sewing jean, denim, etc.....and getting over all those thick spots you mentioned.

You might try this, dampen the material and adjust the pressure foot at a point where you can slide the material under the foot with it resting on the jeans. Maybe your machine is not designed for doing heavy work like jeans?

you need an industrial machine for jeans, use a heavy needle and pliars and do it by hand.
