It's easier to admit that your mistake was someone else's fault then your own. Because then YOU have to deal with the consequences.
Since a lot of people haven't learned from their parents that negative actions cause negative reactions they don't want to deal with the punishment or even the couple minutes it would take to rectify the situation and apologize.

Not all, but some people do have a problem with personal accountability because their parents did everything for them and gave everything to them. When they got in trouble, instead of applauding the parents or law officers for punishing them, the parents were outraged on their precious child's behalf and defended them and soothed their child's feelings and told their child that they would make everything alright.
The movie makers for the longest time cast everyone as victims. I am not responsible for the crimes I commit. My parents are responsible because of the way they raised me. Society is responsible because they did nothing about it.
Money hungry lawyers feed on the lack of personal responsiblity. If I am physically injured because I used a lack of judgment and did something stupid, the lawyers will tell me, "no, it's not your fault. It's the fault of a product designer or the person who wrote the instruction manual, so we'll just sue a large corporation and make them responsible for your stupidity."

Many religions have a problem with Christians and the idea of "the devil made me" or "let go and let God". Be they pagan, Buddhist, Druid, atheist, Taoist, et al. all expect people to take personal responsibility for actions and choices and for inaction when action is required. There are even groups working to revise the AA model to emphasize personal responsibility and making appropriate choices.

Adam said to god "the woman thou givest me."
He tried to blame God for his breaking the one and only rule. (well if you hadn't put eve here and made her my mate, I never would have done it) It's human nature to try and fob the blame off on someone else. One of my favorite office posters is "I didn't say it was your fault, I said I was going to blame you"
It's not just our society, it's every society ever. By not being personally accountable we hope we won't have to face the concequences of our actions.

because it starts with the parents being embarrassed to admit it when they fail, and then, we grow up with that, and then, we are too embarrassed to admit it when we fail! being a teenager, i know that it's especially hard for a teenager to 'fess up when they make a mistake, and then the adults yell at us, even though they do it all the time, too! so not fair!
