One reason is that your clothes, pajamas, etc. are not sterile.
Another reason is that if there is an emergency during surgery, operating room personnel won't bother having to cut away pajamas, underwear, etc. to gain access where it may be needed.

There are several reasons, one is a hygene issue. They need to be meticulous about hygene so not to spread any unwanted infections. If you are having surgery on a specific body part, then this area must be free from any clothing or underwear.
The other reason is that the electrical equipment can interfere with certain materials in your clothes and underwear. The gowns they give you to wear are pure cotton and this will not infere with the equipment.
However, you can sometimes leave your undies on or they will give you some paper ones. I've also worn my bra on several occasions. It depends on what sort of surgery you are having and also the policy at the clinic or hospital you are having surgery.

For the same reason that anyone in the operating room must wear sterile surgical clothes -- maintaining a safe sterile environment. Clothing has bacteria on it, and that bacteria could lead to an infection following the surgery.

Clothes aren't sterile, and would only get in the way. Surgery is a serious thing. Don't be shy, the doctors have seen it all before.

Microscopic organisms. Helps keep the O.R. antiseptic. A hundred and twenty five years ago, what you didn't see could kill you. You aren't the only patient going into the O.R.
