Ohio only has 2 versions of the individual income tax form. The IT 1040 is good for anyone. The IT 1040 EZ has limits. I recommend filing on-line (free at the Department of Taxation website). If you do, you don't have to decide which form to use. The website will figure that out for you. Too bad the IRS can't do the same on their site.
