Make sure you have your cell phone off and there are no distractions around, like the phone ringing, the radio/stereo on, etc. If you can't achieve total quiet, then get a tape with ocean waves or something like that, and play it softly and it will block out the background noise. Make sure you are getting enough sleep, in general.
Study for about 45 minutes, then take a break, call up a friend, get a snack, whatever, for 15 minutes or so, then go back and study another 45. Avoid study groups, in my opinion, there is a lot more chit chat going on than studying.
If you just can't keep from doing other stuff while studying at home, go to the library or go to school early and study, where there is absolutely nothing else to do.
I have to say, for some subjects, like calculus or physics, I just could NOT sit and read those chapters. For me, homework was the learning, I just did the homework, and then MAYBE when I was done, I would go back and work a proof, but mostly not. Pretty much, if you get thru all the homework problems, you really do know the material. For science books, you read the graphs/charts/pictures first, they are more interesting and tell you most of what you need to know. THEN you go back and read the chapter. To be honest with you, I did that for all my other classes, too, if there were pictures, I read the captions first. And guess what, I was a merit scholar and went to grad school for physics, so I didn't do too bad with these methods!! I think people blow a lot of time trying to READ math books and stuff, and that stuff just doesn't make sense to me until I did the homework first. And I NEVER had the radio going.

The lower your stress level, the better your concentration.
Something people don't often think about, concerning concentration, is that our lives are very full and we try to rush through our tasks. I find it helpful if I limit the number of things I feel pressured to perform, so that the few I do, I do well.
Commit blocks of time to your schoolwork. See that you have eaten something and are comfortable. Purpose, when you are noticing you feel distracted, to take 5 or 10 minute breaks - get some fresh air and a cold drink - then resume the studying again and keep at it with a specific goal in mind.
I think you will find your concentration abilities will improve.

Play mental focus type games like hidden pictures and teach yourself the hand, eye, thought coordination you need to be able to focus.
Cut out caffeine and complex carbohydrates after around noon, when preparing for a test and you can also try Ginko Biloba.

Try meditation. It's an excellent way to build concentration skills. You can read the excellent book "Mindfulness in Plain English" online (see sources).
If you want to go the quick and dirty route, there's ProVigil (which is prescription-only) or coffee.
