Do I address the label Mr & Mrs Johnson?
or

Inside = Mike and Mary Johnson

Use full names and titles, it should read:
Mr. and Mrs. Micheal Johnson
if the couple is married,

if the woman has a title but is married is should read like this:
Mr. Micheal and Doctor Mary Johnson

and if the man has a title but the woman does not like this:
Dr. (or Rev...you get the idea) and Mrs. Micheal Johnson

if the couple is living together but not married like this:
Mr. Micheal Johnson and Ms. Mary Schimdt

That is for formal invations if you even is more causal you could say Mr. and Mrs. Mike Johnson and so on.

The "rules" have changed. If you know how the couple would like to be addressed, do that. For example, when I was married, I never thought there was a Mrs. Harvey Brown. Formally, of course, there was. But I was Mrs. Andrea Smth, or Ms. Lucy Sanchez, never my husband's name and our last name. That always sounded odd to me. If you know someone like me, then use that. If you don't, it's not a major mistake to address it traditionally. But if the couple live together, address it both of them seperately, but on one envelope. I have also seen each party, married or not, recieve a seperate invitation. That message was: there is no room for guests, just you.
If you are having a dinner with the reception, then this is more important so you won't have wedding crashers. I knew a man once who regularly ate at the weddings of people he had never met. Always gracious, he just charmed everyone around him, and no one thought to ask. So for a dinner, require the guests have a seperate card with them at the door, and someone to check their card against the guest list, or someone you invite will not have a meal.

The correct way is...
Mr. and Mrs. Michael (if he is one) Johnson
The address should be .........Street, Road, Way... and the state spelled out. No abbreviations.

On the inner envelope you can be less formal, Mike and Mary. Save the... and guest for this too.
