I don't think it needs to be quite so formal. Just something like: Dear Ms. (her name), Due to our religious beliefs, we would prefer if our son, Bob, did not participate in dance at school. He will bring a book to read during dance class (or, we have made arrangments with the principal for him to join another class during that time). If you would like to discuss this further, please feel free to give us a call.

Dear Ms. put name of teacher here:

My wife and I are the parents of Bob put last name here.

Due to our religious beliefs, we do not want Bob to participate in the dance program at school.

We realize the importance of inclusion in various school programs, but, unfortunately, this is one activity from which we cannot allow Bob to participate.

As his teacher, we hope you will make sure that Bob is not ostracized by fellow students, due to our belief and decision.

We hope there will be other events in which Bob can participate.

Sincerely,

Mr. and Mrs.

Yep, you've got it. There really is no set format for a letter like this. You just have to state that you, as his parents, do not want him involved in the dance program. If the school asks why, you should let them know that it's for religious reasons. They do not need to know more than that. By law, they do have to honor your wishes.

Dear teachers name,

It has come to our attention that our son (his name) is required to be a part of the schools dance program. We feel that it would be in the best interests of our child to not participate due to our religious beliefs (or it can be said that it is against our religion, or the religious beliefs can be left out). We wish to remove him immediately. If you have any questions please feel free to contact us.

sincerely,

the parents

PS. we can be contacted at 000-000-0000 at (certain time, after certain time, etc.)

It may also be more helpful to write a letter to the principal along with the teacher. I would use the same format and have the dear line as (schools name principal)

How about: Due to our personal lifestyle we would prefer if our child was excluded from the school dance program.
Because it's for religious reasons the school has to abide. The school should not force your child to take part in something that it again his religion. You have the right to say yes or no on school programs. Good luck.

This is in regards to the upcoming school dance program. Myself and (wife name here) are not comfortable with Bob taking part in the program due to religious beliefs. Thank you so much for your understanding of our wishes. Please feel free to contact us with any questions or concerns you might have.... When dealing with teachers I have found it best to keep it short and direct. I hope this helps.

You don't want him to dance or you don't want him to participate AT ALL? I think you should be specific and just state...

Our son does not have permission to participate in any part of the school dance program. Please contact me if you have any questions.

If you like, you can state that it is for religious reasons, but it is not necessary.

i would start out saying your beliefs and that you do not want your son participating. you need to make clear why you do not want him being involved. you cant just say you dont want him dancing. i would also list some alternatives for him say that instead would he be able to .... blah blah blah. at the end make sure to include contact info. also. the teacher should respect your wishes.
