The phrase "under God" was added in the 50s during the "red scare." It was seen as a way of insuring that the US would never become a "Godless commie nation" during the hysteria surrounding communism.
After the panic wore off and people actually educated themselves, nobody saw fit to remove the phrase again.
As to what part of the ethereal anatomy it refers to... well, I try not to think about it.

That was inserted by the Catholics during the 1950s, along with the motto on our coins.
