Terra sanguinis mei
Sanguis cordis mei
Cor terrae meae

Sanguis, sanguinis and cor, cordis are neuter nouns, so with them it's 'mei' for ''my'. Terra is feminine, so it's 'meae'.

With regard to Rick's comment, 'terra' would mean your personal land - your homestead, local area, region. If you mean it in the sense of nation or homeland, 'patria' would be the choice - put 'patria' in place of 'terra' and 'patriae' in place of 'terrae'.

Can't help you positively, but don't follow 'The Queen's" words - they're Spanish, not Latin.
'Terra' = 'land', not 'tierra'
'Sangre' = blood, yes.
"My land" in the last one, however, would probably be used with "mia patria" = "my homeland", not 'terra' as in just the dirt/plants/landscape
