Follow the directions of the 1040 Form. W2 wages & tips goes on line 7. Schedule C is for self-employed. The form will walk you through all the steps and the final figure will also be add to the 1040 on line 12. There'll probably be other forms you'll need for self-employed like Schedule SE for self-employment tax. Follow the 1040 and Schedule C instructions and you'll be walked through it.

Your self-employment income will go on a schedule C or C-EZ. If the net is over $400, you'll also fill out a schedule SE to calculate self-employment tax. The numbers from the bottom of those schedules will transfer to a form 1040. Your W-2 income will just go on the 1040.

You would probably file a 1040, a Sch C and Sch SE.
