1) Trains generally leave their terminals at the scheduled times. The delays occur as people trying to get on a crowded train keep the doors from closing. This in turn delays the trains behind it.

2) It affects me because it takes longer for me to get to my destination.

3) There is not much the MTA can do about it. You can only run just so many trains in an area of track. Telling people not to hold the doors doesn't work at all. No one believes the "There is another train right behind this one" bit. The police could issue summonses for interfering with train operations but they would need people dedicated to doing that all day in order for it to be effective (and that would be a waste of manpower).
