Matthew 6:6
"But when you pray, go into your room, close the door and pray to your Father, who is unseen. Then your Father, who sees what is done in secret, will reward you."
You do not need to be formal,Like this: "Oh tho Great and mighty Father God" Just talk to him like you would any friend. A good start is "Heavenly Father, Or Father God" then just tell him what you want. He does like to hear Thank you." So I always try to remember to first tell him "Thank you" for the things he has done. Even if it for just giving me this day, then what ever I am praying about, whether it is this meal we are about to receive, Protect our troupes, heal my Dad.Our Pastor etc. then yes I do always end in Jesus name.Jesus tells us "I am the way and the truth and the life. No one comes to the Father except through me." (Johm 14:6) It is really very simple. All prayer is effective, Our Heavenly Father knows what we need.

Well, most people do. You could also begin with "Lord Jesus", "Almighty God", "Lord", "Dear Jesus", "Dear Lord" or "Almighty Father". The whole point to that is just directing who you are talking to. If you wanted to you could use any one of Jesus's names from the Bible. Saying "In Jesus name, amen" is just accepting that your prayer goes through Jesus to the Father. The real secret to a great prayer is heart, and faith. If you are truly sincere in what you are saying and really believe in God, He can do anything. Talk to God as you would a really good friend. Don't beat around the bush about something, He already knows what you need, but he likes you to ask Him.

#1. make sure you have forgiven EVERYONE who has ever hurt you
#2. Thank the Father for his care and love not only for yourself but, for all of those who for one reason or another have not thanked him yet that day
#3.speak to your Father from the heart. This could be a formal prayer even as long as it is from the heart. Many times we do not know what exactly to say and end up saying the same thing over again anyway so the Our Father is the best. Then sometimes we can even give our prayers to our Blessed Mother and ask her to polish them up for us and deliver them as only she can. Just saying "In Jesus name" though, is not enough. You actually have to be in Jesus name and that means you must have gone through the marriage ceremony (Baptism)

True prayer comes from the heart, when the mind has been quieted, and is in a place of calm and stillness. Only when the mind has been quieted, can true prayer take place. A mind that is cluttered with all types of baggage is not effective, especially when ones intentions are focused on communion with God. A closet can be compared to stillness. Jesus gave us a hint when he told us to go to the closet to pray.

It says that the Prayers of a righteous man are effective. No one can be righteous apart from Jesus- so first we have to right with Him. How do we do that? Admit that we are a sinner, and ask Him through His shed blood to cleanse us from our sin, and repent(turn from our sin, back to God), God only hears us through His Son. We do want to honor Him by calling Him Father, and we do what to pray in Jesus' name. However these are not magic words to get what we want from Him. Prayer is not getting God to do our will, it is getting us ready to do His. An effective prayer is one that is spoken in faith, realizing that it is only through Christ that we can enter this fellowship. An effective prayer does not mean we will get everything we want, but everything we need- because the One that we pray to knows us better than we know ourselves and knows what is best.

I don't believe so, it needs to be genuine from the spirit. The Lord's Prayer was given to us as a model. It illustrates how our attitude should be toward God and what kind of things we should be focused on. There are many times when I pray that I talk to God just like I would to anyone who was a close friend. I believe those prayers are effective because they come from the heart.

Yes. Jesus always started with Father.
Jesus said no one can access the Father except through Him. So we have to end with in Jesus' name.

there is no real "effective prayer". You talk to God however you need to, but yes, you kind of do need to address him first. The end would also be good too. But whatever you say in between those words needs to come from you, unless it's a meditational prayer.

No. You don't have to say Father God, you just have to be talking to God for it to be a prayer. You don't have to say amen but if you don't say something conclusive (have a nice day or whatever) it feels a bit openended.
