You can claim a live-in friend or fiance as a qualifying relative (they don't have to be related by blood or marriage, it's just the term the IRS uses) if they meet all the requirements to be claimed as a dependent. They'd have to have lived with you ALL year, not just part of it, and you have to have provided over half of their support for the year. If they had gross income over $3300 you can't claim them. If your relationship violates local law or ordinance even if they aren't enforced (and believe it or not, there are still some places with laws like that on the books), then you can't claim them.
But a "qualifying relative" that isn't closely related by blood or marriage doesn't qualify you to file as head of household, and that doesn't change after six years. So no, you can't claim head of household although you might be able to claim your fiance as a dependent.
By the way, that rule changed awhile back, this isn't the first year for it.
