You don't have to. It depends on the look you are going for with your table setting. The charger is just another decoration.

You should really remove the charger when the before course is served (otherwise the cutting of the main course makes an awful lot of noise) so yes you do need a placemat. You would also be better with a placemat if you're not using a tablecloth otherwise the charger plate will slip around.

I would say it depends on how it looks. Try it both ways. I don't think there's any hard and fast rule on the use of chargers and placemats. If putting a place mat with the settings looks too crowded and busy, perhaps "overlapping" too much, leave the placemat off. If you have a large table and your placemat complements the overall look, use it.
Don't forget a nice centerpiece - take into account how much space it will take up.
Just play around with it and see what looks best. If your dishes are simple and plain, a placemat in a print or contrasting color may be perfect. Conversely, if your dishes are ornate, zany and zesty, or embellished with a busy pattern, the settings may be fine without anything else.
