How do I ask a girl out?

I'm not the hansomest man. I would be if I didn't have a cut on my neck that didn't heal right... It's a long scar with stitch marks on my left side. Plus minor acne scars under my jaw. Other than that I'm perfect.

There is this girl who seems to like me. I'm caucasian (U.S.) She is a Japanese immigrant. We're in lv nv I think she's great. And we've been friends for almost a year. Not close friends but, you know. Anyway, things were really starting to work. When all of a sudden, yesterday night, I called her and she was really sick and at the hospital. And she informed me that her boyfriend drove her there and left his lights on. Their battery was dead and they were stranded all day. I didn't know she had a boyfriend untill then, and I gave them a jump. Thus meeting her bf. He has more muscle than me but a lazy eye.

She text messaged me late that night and said she and her bf didn't know each other very well. So again, maybe she likes me.

How do I ask her out?

P.S. I've taken her out before but don't know how to ask her to be my girlfriend.

Ok asking her to be my girlfriend is too childish..... How do I go about kissin' her?

or atleast How do I restart the relationship.

Dude ask her... but say something like...hi .... i find ou to be an interesting person ...and i would like to get to know you better...would you want to do lunch.dinner sometimes. that way you can ask her as a friend /date. both at once. Then pay for the food.
