From an administrative standpoint, the Chief of Staff would be the highest. Many hospital medical staffs elect a doctor to this position for a year or two at a time.
Each department also has a chief: Medicine, Surgery, Pediatrics, Obstetrics/Gynecology, etc. In larger facilities, there may be chiefs of subspecialty departments as well - general surgery, neurosurgery, orthopedic surgery, for example.
From a non-administrative standpoint, the highest position a doctor can achieve is to be one-on-one with a patient, delivering good medical care. It's not about status. Those in positions of power often find themselves doing less and less of what they went into medicine to do.
Hospital CEOs are typically businessmen, not doctors.

The doctor in a hospital can be the Chief of Medical Staff or a Medical Director. Both represents the head of the entire medical team of the hospital depending upon the hospital setting.

Technically, CEO. Medically speaking - Chief of a medical discipline within the Hosp. Chief of Surgery...Chief of Emergency Medicine.....etc and like that.
