I need to send my state thier money for taxes and I am apparently supposed to send it with a payment voucher. I can only find one form online that is a payment voucher, but I can't tell if there is a seperate one for federal and state. Is there two forms or just one? Where do I send it, to the address that is on the form for my state?

Is it for Payroll taxes (941 Form), or is it estimated tax payment? Search www.irs.gov for payment vouchers and directions for forms.

Separate vouchers for federal and state, with separate checks and sent different places. Federal one goes to the IRS, and there are several different addresses depending on what state you're in and what version of 1040 form you used, The state voucher and check go to the state.

Go to irs.gov for federal voucher, to your state website for state voucher, if you don't already have one and need one. If you used a tax preparer and owed, they should have given them to you. If you used tax software, it should have let you print them out.
