This is tricky to find the truth about. Aliases are usually involved in underhanded activity but I have found a couple of boxers who have had at least two.
Light Welterweight Verdell Smith has faced one opponent, Buck Smith, 8 times
twice under his own name
twice as Tommy Bowels
and four as Tim Brooks
Middleweight Reggie Strickland fights about once a month also under aliases: Reggie Buse and Reggie Raglin. his record on boxrec is 66-276-17 with 14 KO's. Im wondering how he is still living. I have yet to find anyone who has revealed more than 2 other aliases but I'm sure they are out there.

There was a guy called Fidel Castro Smith who was british supermiddleweight champion about 15 years ago. He also fought sometimes under the name Slugger O Toole. I remember once he was ranked by the WBA (i think) at no 12 and no 10 in their world rankings. So the same bloke was ranked twice in one set of rankings which looked a bit strange, and was probably quite embarrasing for the people who did the ranking system. After that he seened to drop the Slugger O Toole name and revert back to Fidel Castro Smith.

I know A couple of boxers who used two aliases. Roberto Duran, Evander Hoyfield, and Thomas Hearns. Now you can try boxingrec.com and take a look at different boxers and get other aliases, I just remember those boxer's names.

I would have to say Apollo Creed in Rocky V before he fights Drago the ring announcer goes through about 6 nicknames. My favorite is "the Count of Monte-Fisto".

Many boxers fight under different names (and records) to get fights and get around mandatory rest periods after having been knocked out.
Many bands perform under different names to get around bad reviews!
