By assuming you're a young single the best would be you date a native English speaker and spend with her mostly of yr time. In addition to this read books and newsapers, listen radio and CD, watch TV and movies (all above strictly in English). In this meanwhile speak as less as you can in your mother tongue..
As to overcome your fear, I've become fluent in 3 languages besides my native Italian idiom just by thinking that if I'm speaking with other people in their native language it means I'm more fluent than them in my language. I've never worried about possible mistakes since also native speakers are making a lot of errors (as you can see by reading several of Yahoo's Questions and Answers...!!).
What has been always important for me, and still it is, was to be able to understand and make me understood in my best possible way. That's it....!

You should talk in English without fear but make sure have some close friend to help you iron out mistakes. You will gain confidence slowly and that would help you speak fluently in the future. Best of luck and I am sure you would be speak good english soon.

I'm not a native speaker too so i try to speak with myself loudly in my room and it would be more effective if u try to speak in a mirror looking at ur eyes.
Thats my suggestion.

Try reading more English texts. Enhance your comprehension and widen your vocabulary. You may also try to read aloud or talk to yourself in the mirror. Though it sounds stupid, but guess what, it does work.
