Is this a language question? For example if I raise my kids to call the color people know of as blue, red and if everyone in town does the same are we wrong or are we developing a new language?
Or is this more of a psyche question about who we are emotionally or mentally?
Either way it is an interesting thing.

Boy and girl are labels based on the presumption that you are either one or the other. Being labeled a boy or girl is based on having a certain anatomical appearance. These labels discount the small percentage born with questionable sexual organ appearance as well as labeling based solely on physical aspects while disregarding mental aspects.

I think it probably is the same way that you know when you have done something that is good, or bad. You just know. If you have doubts about knowing, then you probably are what you are.
And no one else can tell you that you were wrong about that.

You mean, how do you know if you're transsexual? I think that you just know. I asked one of my friends (who's a F2M) and he said that he just knew something was wrong when he was little. He felt out of place in his body. I think he even asked his parents if G-d made a mistake and he was supposed to be a boy.
So I think you just know. Your mind will tell you if something's off with your body.

How would I know I'm a boy? If I had a penis.
How would I know I'm a girl? I have a vagina, boobs, a monthly period.
What if you told me I was wrong? I'd show you my vagina, boobs, and tampons, thrown in the trash.
