Ask your workplace for another copy and ask the IRS for an extension.

You need to contact your employer ASAP and since the deadline is Monday, do this
1) Locate your final paycheck of the year if you have only worked for one company at the end of last year.
It will give you your total salary, ssi payments etc.
2) Use that to calculate your taxes. If you are doing it yourself. You can send in and ask for an extension. If you owe money, pay the estimated taxes. The extension is just for the paperwork, not the taxes. If you are e-filing, there would be no worry. If you using a tax preparer such as H& R Block, you will need to file for an extension

File for an extension immediately...and contact your job to get new ones...they might charge you.
