Let say you let a Woman borrow your car and she calls you and says "I hit something but don't worry it's just a scratch" what does that mean to you? To me it means that the bumper is hanging off and the hood is missing.

same here.. i heard that once and went to look at it and my back bumper was all warped like a piece of tin foil..

I think she's trying to let you know an accident had occured but before you have the chance to worry a bit more, she said that it's just a scratch. Or maybe she said it's just a scratch so that you won't be angry at her. I think something bad happened to that car, lol. No offense!

It means that she is honest to even tell you about it if it really is just a scratch. She probably feels really bad.
