I begin a teaching course in September and I need to know all the treatment I have had in the last 3 years. I am in England, UK.

Just ask your house doctor. He will supply you with a relevant copy of your medical history. Be sure to tell him what it's for.

You can simply ask "Records" for your medical history, but expect to pay through the nose for all of the papers. If you are simply transfering from one Doctor's Practice to another there is a medical release form you fill out at your new Doc's office that they will then fax over to your old one (you may have to snail mail it yourself, not sure internationally). Then your old Doc will send all of your records to your new one without you paying and arm and a leg.

In the US all a person needs to do is ask their physician for a copy. I would be surprised if it was that much different in the UK.
