There is no instance or context to call a female officer "sir", or a male officer "ma'am". This is the same in all branches of the US services.

Male officers are addressed as Sir and female officers as ma'am in the US Army. Although all warrant officers can be addressed as mister! Sometimes they must face battles so intense that there's just not enough trees for the officers to hide behind!

Just the dyke officers. There are many. It's OK to say either "sir" or "ma'am" since it's difficult to tell which gender they are.
