Is there ever an instance or context when "sir" is used for any officer of superior rank, regardless of gender? Is the same policy in place for every branch, or does it differ slightly among them?

Enlisted personnels address male commissioned officers as "Sir" and female commissioned officers as 'Ma'am". Also, junior commissioned officers address senior commissioned officers as such.

There is no instance or context to call a female officer "sir", or a male officer "ma'am". This is the same in all branches of the US services.

Male officers are addressed as Sir and female officers as ma'am in the US Army. Although all warrant officers can be addressed as mister! Sometimes they must face battles so intense that there's just not enough trees for the officers to hide behind!

The male officers are all called sir if you are enlisted. Ma'am if they are female officers.

Just the dyke officers. There are many. It's OK to say either "sir" or "ma'am" since it's difficult to tell which gender they are.

We in the US Army were instructed to adress an Officer by his or her rank or Sir / Ma'am.
