Yes, it is something to worry about if it is left untreated.

When germs enter the middle ear, the area fills up with fluid or pus. An untreated ear infection can progress to the point where the fluid build up causes the eardrum to rupture. Of course, there are other complications:
a chronic infection
enlarged adenoids
mastoiditis (infection of bone behind the ear)
temporary or permanent hearing loss
meningitis (infection of brain membranes)

It is always best to have an ear infection treated to be on the safe side.
