Another option you may consider is to do your taxes online. I use www.taxactonline.com to file mine. it costs about 15 bucks i think to do federal and state and that price includes electronic filing. Ive used them for the lasty 3 years and am very pleased

You can use the www.irs.gov site to get all of the tax forms that you need... but why print up the forms when you can e-file?! You get your money back faster when you e-file anyhow... It makes no sense to snail-mail your tax forms unless you owe money!
Use the top URL to get your return within 24 hours.
Use the bottom URL if you owe money or back taxes to see how much money they can save you.

ALL IRS forms are available from the IRS at the link below. I don't think there is such a form as 1040-B.

www.irs.gov can give you the federal return forms.. may have to go to post office or public library for the state forms... good luck, and get going.. time's running out! :-)
