the supervisor should be the one addressing the mistakes, not you.

If you are on the same level as the co-worker, then your input would be seen more as a personal assessment of the worker. Professional assessment of job performance should come from a higher up.

It's not your job to keep everyone aware of mistakes unless you will be held responsible for those mistakes in the long run. When it affects YOUR performance rating, then say something to them.

If she thinks she's great and she keeps making mistakes, you have a very emotional volatile co-worker. If you approach her, she most likely will cause a scene, since she's extremely vain, and most likely a drama queen. Proceed with caution. Don't tell her she made a mistake. That would be a big mistake. Let the boss discover the co-worker's mistake. Plant it on the boss's desk without leaving any trace that it was you who left it on the desk. Be secretive. It's the only way to survive.

Just say, I noticed when you do whatever it is, that sometimes you forget to do this. Or just say, I need some clarification about this, then when she explains what she was doing you could offer help, like in the office we usually do this so we all know what it means. I don't know what setting you are working in so my advice may or may not be helpful... Either way good luck.

Why should you tell her? Leave that to your boss. You don't want to make enemies at work and unless it'll somehow make you look bad, let the boss handle it. If you do need to say something put it nicely like "hey Pam, these numbers don't add up...I think you need to recalculate them".
