This is a line from Silence of the Lambs. What the character is trying to say is that the girl needs to put the lotion on her skin in order to soften it up, or he will turn the hose on her and cover her with water again. This is a form of torture to have her do what he wants her to do.

Its a reference to "silence of the lambs", the crazy guy is instructing his captive to do this in a narrative style to depersonalise her. The lotion is to treat her skin so it makes better quality leather for him to wear (budget sex change). The skin thing is based on Ed Gein but he only killed 1 or 2 people, most of his furniture was made from dug up corpses.

It means that if the woman in the hole doesn't moisten up her skin in order for it to be used for a new coat, then she will be hosed down by her captor until she follows his directions.

He wanted her skin soft, and she wasn't cooperating, so he was trying to force her to do what he wanted.
