Religion is the main problem. Most human rights issues promulgated by religion from a time when human rights wasn't an issue, because there were none for most.
The solution to the problem of religion is just education. This is probably why religions are so keen on controlling the schools. I think that, deep down, they realize that their only chance for survival is indoctrination of children. Religion is not something that really stands on its own.
Luckily, I think that more and more people are realizing that religion is just old superstition.

Not having the proper access to clean water.
Teach those in need how to clean unclean water, or make the top five nations (in terms of GDP) work on helping fund well foundations.

Problem: Abortion's legality or not
Solution: Allow abortions to be performed in the cases of rape or other sexual crimes, but not for someone who just "changes their mind." If you feel you can't raise the child, put it up for adoption.
