Here's the link from the IRS site. Just click here
http://www.irs.gov/pub/irs-pdf/f1040ez.pdf?portlet=3 and you will see your 1040EZ Form and then you can just print it out! So easy to do or you can even file online which is even more simple because you are not mailing any forms in at all and you will get your refund even faster, if you are due one! I posted the link below if you want to try it.

Sounds like a 1040 standard. Three forms 1040EZ, 1040A then 1040. Filing Schedule A with 1040 souds like your deal.

For the best answers, search on this site https://shorturl.im/1aFFC
You should be able to file regular 1040. If you have over $10,700 (standard married filing jointly deduction) in mortgage interest, property taxes, state taxes, medical expenses over 7.5% of your salary, donations as a total you need schedule A, otherwise you do not. You need Schedule B if you have over $1500 in interest or common dividends. You can try itemizing on a Schedule A to see if it benefits you. This year it does not benefit our family since we have very low property taxes and low mortgage interest (thank goodness!)

I would always see them at the library or the nearest tax office. I think online you find it now.
