FUO "fever of unknown origin" is always a medical challenge, but your mention of impaired renal and hepatic function go beyond normal FUO considerations. Are you in hospital??? First; I would get a blood test for acetaminophen levels as toxic levels could be causing the organ failure. Secondly; get infectious disease specialist involved in the case. Finally get a pharmacist consult to check for possible drug induced fever. I had one patient with ongoing FUO that we finally discovered was caused by a reaction to her Lasix.

Your mention of CVA does posit other considerations, but I have assumed that your neurologist/ primary care M.D. have already taken these into account.

Rule out pontine hemorrhage.If present pontine hemorrhage needs special attention. Vital parameters are to be kept constant with symptomatic treatment .Fluid intake and output need watch. Fever disappears generally with antipyretics.
