MACINTOSH OSX adding an account in system preferences.?

When I launch the "accounts" pane in OSX 10.3.9, to add an account on this computer (G4 533DP) but when I click on the "+" buton, no new account is added... any ideas?

even with the lock open.

Click on the Padlock and enter your Admin password.

Then, the "+" sign will work.
