You do not have to go to the sacrament of Reconciliation (Confession) before every time you receive the Eucharist (Holy Communion).
Catholics should receive the Sacrament of Reconciliation whenever he or she has completely cut oneself off from God (in the state of mortal sin) and at least as once a year.
Catholics may receive the Sacrament of Reconciliation whenever he or she feels that they need to be reconciled to God and the Church.

You must convert- or be in the process of converting- before you may participate in the Sacraments.
A person who will be baptized when they enter the Church has no need to go to Confession, because their baptism will wash away their sins.
A person who was already baptized and is becoming Catholic will go to Confession before they actually are Confirmed.
All Catholics should be in a state of grace ( free from serious sin) before they receive the Eucharist. St Paul tells us that those who receive the Eucharist unworthily are "guilty of the Body and Blood pf Christ."

It is normal practice for confession to precede communion as a sacrament. It is not necessary to go to confession before 'every' communion. One cannot receive communion in a state of 'grave' sin.

If you believe yourself to be in a state of mortal sin, you must confess it before receiving Communion at mass.
You can still receive Communion in spite of venial sins. As a matter of Truth, your venial sins (assuming you repent) are forgiven as you receive Communion.

In order to validly receive Jesus Christ in the Eucharist, you must:
1) Be baptised
2) Be in full communion with the Church
3) Not be in a state of mortal sin
If you have a mortal sin on your heart, do confess that before receiving communion!
a) You'll get that off of your soul to begin with
b) You will be able to receive Jesus with a pure heart. St. Paul reminds us to discern ourselves, lest we eat and drink judgement upon ourselves. If we discern mortal sin, we should not receive the Eucharist until we have confessed.
c) You receive grace SPECIFIC TO THE SINS YOU CONFESS when you go to confession. So say you confess a sexual sin; you'll receive the grace to be chaste in the future. Good thing--that's a lot of chastity grace for me. :-/
d) There is no sin in the history of the universe that is beyond God's mercy. We just have to ask for it.
Remember: Not all "grave sins" are mortal sins. For a sin to be mortal it must:
a) Be a grave sin
b) Be done with full consent of will
c) You must have full knowledge that it is a grave sin
You do not have to confess venial sins prior to receiving Communion; on the contrary, it takes away venial sin! Of course, it is always a pious act to go to confession for venial sins.

yes, you cannot recieve holy communion if you have commited serious sins, you must give a general confession to the priest first.

I had to go to confirmation when I was a kid, before I was able to receive communion.
I'm an atheist now, so it's probably another set of rule at this point.
