It can mean "ready" or "clever", depending on the context. When it's used with the verb "estar", it means ready. When it's used with the verb "ser", it means "clever" (as in "smart").
Examples:
La película va a empezar. Tenemos que irnos ya.
¿Estás listo(a)? =
The movie is going to start. We have to go now. Are you ready?
Se te ocurrió muy buena manera de sacarnos de ese lío. ¡Eres muy listo(a)! =
You came up with a really good way to get us out of that mess. You’re very clever!

Two meanings:
Clever = Listo (smart guy). Ese chico es listo.
Ready = Listo (are you ready?) Estas listo?

listo means ready or done. You can use it when cooking saying: The chicken is done: El pollo está listo. We are ready to go: Estamos listos para salir.
It is also used as an adjetive meaning street - wise or inteligent like: El es muy listo. He is street wise.

It has two meanings, be ready and be clever.
Those, there is a third variant, when you say "estás listo", it can be mean as well "you are in trouble", or something like "you can wait but aint gonna happen".
Examples.
Juan es listo= Juan is clever.
Juan está listo= Juan is ready.
Juan, estás listo si crees que Maria va a venir= Juan, Maria aint comming, you are waiting for nothing.
Juan, te vi rompiendo la ventana, estás listo= Juan, I saw you breaking the window, you are in trouble.
