wnat one that can have the names and date added

Why don't you just make them in Microsoft word? You can get all kinds of interesting paper from your local office max or Art store (Archivers has cool stuff too) From there if you want to get fancy you could have them matted and framed or buy frames and do it yourself.
Or you can take a picture of the child to Kinko's or something similar and see if they'll photoshop something for you like a banner or calendar whatever.

Why don't you make one yourself? You can find templates and designs on the net, or make one from supplies from a craft store. Or even a shadow box, with a pic of the baby then the dates etc., much more personal, and meaningful than a generic certificate.
