Depends on your context with him. If he's a co-worker or someone you will see frequently, you may need more tact, than if it is unlikely you'll ever run into him again.
Don't lie about seeing someone else. If it is exposed, it's a reflection on you.
Be as truthful as you are able without being insulting.
If the guy is not a total loser, remember hygiene and weight are not absolutes, and can be remedied.
If it is not too embarrassing for you to find a polite way to tell him, do it.
Or ask another guy to tell him. In the long run you may be doing him a great favor. He could clean up quite nicely.

Just tell him you are not interested. I think it is very polite to be upfront with people and let them know where they really stand. You don't have to tell him why you are not interested (that is none of his business) only that you are not.
