the answer to "how do you do?" is the same "how do you do?"... this is only used when you are introduced to another person as in:
Frank: "John, this is Mr. Smith"
Mr. Smith: "How do you do?"
John: "How do you do?"
so it's not an everyday greeting as "how are you?"...you will only use it the first time you meet a person and it's more commonly used in England

"How do you do" is a formal greeting that people say when they are being introduced to one another. It is relatively rare and old-fashioned, and the correct response back is "How do you do". It is not a question asking how you are, so all the people who responded with things like "fine" are wrong!

The response is "Fine, how are you?" If you take the greeting literally, you would recount your aches, pains and whatever else keeps you from being "fine". But people mostly mean it as a greeting and don't really want to know how you are. :>)

The correct answer would be to repeat it back.
A) How do you do
B) How do you do
But it doesn't really matter because you'll never hear anyone ever actually say it.

"i'm fine, and you?" is an option.

Here in Australia we DON'T say 'how do you do..', it's soooo English......
we say 'How are you?' and you should generally respond with a simple 'good thanks, and you?' :-)
