If you know that you are getting a refund, then you will not be penalized and owe any fines if you do file after April 17th---but if you do owe money then you must file by that date to avoid penalities.

You actually have up to 3 years to claim any refunds so technically you could wait and you do not have to file for an extension to do so. But if you do want to file an extension anyway just in case, the Form number is 4868 and you can download it here:

You can't get your refund until you file your return. If you're not going to make the 4/17 deadline, request an extension - see http://www.irs.gov/pub/irs-pdf/f4868.pdf?portlet=3 for the form and instructions.

you have three years to file for a refund. however, if you suspect you even owe a single penny, file the form for automatic extension as it will keep the irs from asking about you.

Call the I.R.S. and ask for an extension and they will tell you what forms you need..
