A manual transmission is some times called a gear box.
As for pinion,
Rack and pinion as in the steering box?
Or the pinion gear in rear-end of a rear drive vehicle?

gears are circular for speed increase/reduction (a car has different gears for different speeds)

rack and pinion are straight and for gripping/stability (some trains mounting the hills are with rack and pinion arrangement

the big difference is the gear ratio, that is just about it aside from the lay outs.
