If you know the county and state you were born in, you can contact the Department of Vital Statistics for that state and make a request for a certified copy. There is usually a fee (less than $50) for this service. Contact your local Department of Health and Welfare for address and futher information.

You need to go to the city or county where you were born to get a new one. If you still live in the same place, look up the office in the blue government pages of the white pages phone book. It should be the office of vital statistics. It may just say birth certificates. You will need to go in person and bring lots of valid picture ID to get a certified replacement copy as well as money.

You go to the Clerk of Courts in the county you were born in. Don't live there, mail them to find out the fee
