Do you actually check the notes you did about the class during it? If so, rarely? Occasionally? Frequently?

It depends. In the beginning I used to use them religiously, then as I became more confident I found I could either improvise or knew the topic well enough. But even now if I have a new idea on teaching sthg or a new topic I keep the notes on the desk and am not ashamed to refer to them - better than making a nasty mistake trying to teach kids the concept of perfect tenses when they have no such thing in their own language :)

When I started teaching (many years ago) I often looked at my notes. Now it's not necessary to follow them so exactly, if at all. I have learned to be much more flexible than I used to be.

In the beginning - frequently. I read through them thoroughly before the lesson and kept reffering to them throughout.

.Rarely! I think that comes with years of experience.

As you gain confidence in what you are doing it is easier to wing it.
