On your divorce papers there is a spot to request your name back. When the divorce is final you take your divorce papers in and get all you information changed, like drivers license, social security card, etc.

Depends on the family court where you live. Here in California the first form you fill for marriage dissolution has a check box for the woman to change back to her maiden name and a space for that name.
Check with court resources, look at the forms, and do some research online for you particular state

when i got a divorce, that was part of it....i had the option to return to my maiden name or keep my married name....i kept my married one because i had kids and wanted us to have the same last name

on your petition for divorce you can note your intention to change.when the divorce is final go to the social security office and file a name change,go to your secretary of state and have your liscense changed and your bank any place that is important you should notify including your phone company electric ,gas,cable etc., good luck.each place has different requirements .

You can change it back in the divorce papers. You will have to get all new ID again but there it is

there is a section on the divorce paperwork requesting you indicate if your keeping your married name or switching back to your maiden name....at that point indicate what its going to be and once final ...it will be legally transfered.

You only have to ask for it to be restored in the Petition and then again in the Decree. There is no extra charge to change your name as long as you do it at the time of the divorce.

Call the social security office, and find out the exact steps you need to follow to change your name. After this is done, you can go to the DMV and have it changed on your driver's license/ID card. Then change it everywhere else (bank accounts, credit cards, etc).

in the divorce papers state you wish to have your last name prior to your marriage reinstated. It happens all the time.
